For those born outside of the United States:


You will need to provide your ORIGINAL birth certificate along with one of these documents to enroll in Medicare:


  • Proof of citizenship (Naturalization Certificate or Passport)
  • Work visa/permit showing you are authorized to work in the USA
  • Green Card showing permanent residence
